Total Wellness Centre is committed to the on-going protection of your personal information. The information we collect from our clients allows us to ensure your health care needs are met or exceeded.
What do we collect? Total Wellness Centre collects personal client data directly from our clients, which data is generally limited to name, home and business contact information, date of birth, billing, employment information, medical information and history, all of which is necessary to provide clients with safe and effective treatments.
How is this information used or disclosed by Total Wellness Centre? In order to protect you, we will not collect, use or disclose your personal information for any purpose other than those that we identify to you and with your permission. All records are kept in a secure area with strict access control and are shredded when they are no longer required. Your personal information is only used to complete the tasks which you have agreed to, such as collection of unpaid charges. In the case of an emergency, when necessary, we will provide appropriate personal information to medical professionals.
How you may change your information or withdraw your consent to the use of your information? At any point you may withdraw your consent, refuse to allow us to obtain, use, or share your information, subject to legal or contractual restrictions and considerations. You may also advise us of any updates or changes in your personal information, which will help us serve you better.
Electronic correspondence: Total Wellness Centre cannot guarantee the privacy of electronic correspondence. Although we do not allow access to our computers and any printed materials to anyone other than the staff of our clinic, due to the nature of electronic correspondence, privacy cannot be guaranteed. All electronic correspondence is therefore deemed insecure. DO NOT SEND CREDIT CARD INFORMATION VIA EMAIL, or any other information you wish not to be shared without consent.
Openness: Our Privacy Policy is available to clients and the general public. The Privacy Policy may be updated regularly without notice. General Office Policies
General Cancellation Policy
If you cannot attend your scheduled appointment, please notify us by telephone as soon as possible. If you miss a scheduled appointment a hole is created in our schedule. Patients who have acute problems or need to be seen the same day may be inconvenienced or denied treatment.
We ask that you provide at least 24 hours notice of a change or cancellation of an appointment. Missed appointments without 24 hours notice will be charged at the full treatment fee.
After-Hours or Off-Site Appointment Cancellation Policy for IVF-IUI Patients
IVF/IUI patients who must cancel an after-hours or off-site appointment for must provide at least 24 hours notice. Missed after-hours or off-site appointments without 24 hours notice will be charged an administrative fee of $50 plus HST.
Refunds
There are no refunds on the sale of herbal prescriptions. Each prescription is unique and prescribed to be used by one patient alone. We follow the guidelines as set out by the Ontario College of Pharmacists and the College of Traditional Chinese Medicine Practitioners and Acupuncturists of Ontario (CTCMPAO) which stipulate that prescriptions cannot be returned.
There are no refunds on pre-purchases or packages of services or products.
Patient Appointments
Please try to arrive 10 to 15 minutes early for your appointment. We are very careful in keeping our schedule on time, so that it is unusual for you to be seen more than a few minutes past your scheduled time. Arriving early allows you to rest a few minutes. This allows your pulses to calm for the visit. When a patient rushes to make an appointment, it disturbs the qi and pulse. We want to make sure that you benefit fully from a complete treatment session, and this is another reason to come a little early. Feel free to help yourself to some fresh spring water or ask the staff for a cup of tea while you relax.
If you are attending your first appointment, you will be asked to complete some forms, in addition to any forms you may have pre-filled and brought with you. These forms may take several minutes to review and sign prior to your consultation. For your first appointment, we ask that you attend at least 15 minutes before your scheduled consultation time.
In order to be fair to other patients, we do not extend the treatment into another patient's appointment if you arrive late.
Payments
All payments are due immediately following your treatment. We accept cash, Visa, Mastercard, and Debit Card. All goods and services are subject to applicable taxes.
For your convenience and if you wish, we can securely store your credit card information for future charges. A form will be given upon intake to this end.
